Welcome to the NARBHA network! Because people from all walks of life sometimes need help paying for the healthcare services that keep them safe and well in our communities, NARBHA works with healthcare providers throughout northern Arizona to deliver these services with a conscience.
We continuously enhance our behavioral health and primary care provider network by increasing consumer choices when receiving services throughout northern Arizona. NARBHA continues to expand programs, services, and providers to strive to meet people's needs within their own communities.

Health Choice Integrated Care (NARBHA) offers the following resources to help our community with the recent tragedy at Northern Arizona University:
- Our 24/7 Crisis Line is available at 877-756-4090. We are prepared to provide crisis evaluations and support immediately. Mobile crisis services are also available and can be accessed through this number.
- Northern Arizona University has established an information line at 928-523-0007, which is able to provide general information and status, and help direct family members, as needed.
- 24 hour Walk-In Crisis Center for behavioral health emergencies is available at The Guidance Center, 2187 N. Vickey Street, Flagstaff, AZ 86004, 928-527-1899.

We're with you all the way
We will help you choose a provider from within the provider network. You will need to contact the provider to make, change, or cancel your appointments. If you need help with scheduling your appointments, contact NARBHA Member Services. If you are not happy with the provider you chose, contact NARBHA Member Services to discuss the situation.
If a provider leaves the NARBHA network, NARBHA provides written notice of a termination of that contracted provider within 15 days after receipt or issuance of the termination notice to each member who received behavioral health care from, or was seen on a regular basis by, the terminated provider [42 CFR 438.10(f)(5)]. Affected members are informed of any other changes in the network 30 days prior to the implementation date of the change [42 CFR 438.10(f)(4)].
